The company’s accountant receives an instruction from his managing director authorising him to make an urgent payment to a third party organisation. The instruction arrived by email whilst the MD was away from the office and due to the urgent nature of the request and the apparent genuine instruction, the payment was made.

But following the MD’s return he advised he had not sent the email or instructed the payment authorisation, but by then it was too late, the payment had left the company bank account.
